Passport Application

DVSE is making available three forms from the US Post Office website.

The first form is for a new application.
The second is for renewal of an existing passport.

The third document is a special consent form for minors under 16 - Special Circumstances - see below for more details if you need to fill this out.

• You are the non-applying parent or guardian providing written consent for your minor child to apply for a U.S. passport (because you are unable to appear in person with your child) or
• You are the applying parent or guardian, and the written consent of the non-applying parent or guardian cannot be obtained

Please make three (3) copies of your passport and mail to
